Members and covered dependents can make an appointment with a CampusCare Primary Care Provider at the locations listed below without a referral or preauthorization. CampusCare Primary Care Providers are responsible for coordinating covered Members’ healthcare needs and maintaining medical records. A CampusCare Primary Care Provider is the first person a Member should contact whether for routine care, illness, injury, urgent or Emergency Care.
**All Members must make an appointment with one of the CampusCare Primary Care Provider at one of the locations listed below to receive authorization or a referral before seeking care with a specialty physician in order to be covered.All members must establish a provider relationship prior to any referrals or treatment. This can currently be done remotely via a telehealth visit.
Unauthorized services provided outside of the primary care provider clinic are the enrolled Member’s full financial responsibility.
